REQUIREMENTS FOR ENROLLMENT

For Universal Pre-K, your child must be four (4) years old by December 1, 2024, and the family must reside within the Greenburgh Central School District’s boundaries.

For the Head Start program, your child must be turning three (3) or four (4) years old by December 1, 2024, and qualify per the federal guidelines. If you live outside of Greenburgh Central School District boundaries, you must transport your child to and from school.

Head Start eligibility according to federal income guidelines requires one or more of the following documentation:

  • Four consecutive paychecks, W2, a letter from the employer (If family income falls within the Federal guidelines, see chart found here.)
  • Unemployment statement
  • Proof of TANF eligibility
  • Proof of SNAP
  • Proof of homelessness or proof of shared living | Temporary housing
  • Proof of SSDI benefits for anyone in the household
  • Foster child

Para Universal Pre-K, su hijo debe tener cuatro (4) años antes del 1 de diciembre de 2024 y la familia debe residir dentro de los límites escolares del Distrito Escolar Central de Greenburgh.

Para el programa Head Start, su hijo debe cumplir 3 años o 4 años antes del 1 de diciembre de 2024 y calificar segun las pautas federales. Si vives fuera del Distrito Escolar Central de Greenburgh, debe transportar su hijo hacia y desde la escuela.

La elegibilidad para Head Start de acuerdo con las pautas federales de ingresos requiere una o más de las siguientes documentaciones:

MEDICAL REQUIREMENTS

Please be aware that there are several medical requirements needed for your child to enter the Early Childhood Program:

  1. A medical history will need to be supplied at registration, and there will be several consent forms to sign.
  2. Please bring your child's immunization record with you for registration. It must be completed and signed by a physician. It should include:
    • Measles, Mumps, Rubella (MMR): 1 dose on or after the first birthday
    • Diphtheria, Pertussis, Tetanus toxoid (DTAP): 4 doses
    • Polio (OPV or IPV): 3 doses
    • Hepatitis B: 3 doses
    • HIB: 3 doses before 15 months or l dose on or after 15 months
    • Pneumococcal (PCV): 4 doses by 15 months
    • Varicella: 1 dose on or after 1st birthday

All immunizations must include the month, day, and year given.

  1. Physical form - Completed, dated, signed, and stamped by a physician. It must be within the year. A new physical will be required annually from the date stamped on the current physical. A PPD Tuberculin test or a physician's signature shows that the child is not at risk. Also required are blood tests for lead and hematocrit levels. These must be documented with the results.
  2. Dental form - Completed, dated, and signed within six months of admission to the program.

Tenga en cuenta que existen varios requisitos médicos necesarios para que su hijo ingrese al Programa de Primera Infancia:

  1. Se deberá proporcionar un historial médico en el momento del registro, y habrá varios formularios de consentimiento para firmar.

  1. Formulario físico: completado, fechado, firmado y sellado por un médico. Debe ser dentro del año. Se requerirá un nuevo examen físico anualmente a partir de la fecha estampada en el examen físico actual. Una prueba de tuberculina PPD o la firma de un médico muestran que el niño no está en riesgo. También se requieren análisis de sangre para niveles de plomo y hematocrito. Estos deben ser documentados con los resultados.
  2. Formulario dental: completado, fechado y firmado dentro de los seis meses posteriores a la admisión al programa.
